The Mainstreaming on Main Street® (MoMS) Community is an opportunity for persons with disabilities to engage with others for the purpose of personal and professional development.
We are building a network of people who care about creating communities where individuals with disabilities can fully participate, grow, and thrive. Whether you are a person with a disability, a family member, caregiver, friend, or advocate, your perspective matters.
We invite you to complete the Mainstreaming on Main Street® (MoMS) Community Profile, a brief survey (about 15 minutes) that will help us understand your interests, experiences, and priorities. Your input will help shape a stronger, more connected community that reflects the needs and goals of the people who live in it.
Why Take the Survey?
Share what’s important to you and your loved ones
Help identify the kinds of services, supports, and issues that matter most
Join others in building a network focused on inclusion, access, and well-being
Choose how you want to be involved—whether learning, sharing, or connecting
What’s in the Survey?
You’ll be asked about:
Your connection to disability (personal, family, caregiving, etc.)
Topics that matter to you—such as accessibility, employment, education, housing, transportation, recreation, and more
How you’d like to engage: whether learning more, helping others understand, or working with others to improve your community
Everyone’s voice is important. Whether you’re new to these issues or have years of experience, you are welcome here.
We hope you’ll take a few minutes to complete the survey and help shape the future of the MoMS Community.
Thank you for being part of this effort.
